To ensure a safe injection schedule, a travel questionnaire must be fully completed before any booking request can be accepted. During the online request process, you are required to submit complete patient details, including your Full Name, Full Address with Postcode, Date of Birth, and NHS Number.
Once our pharmacy team reviews your medical and travel logs via our dashboard, we will approve or decline the appointment, ensuring you receive the appropriate booster tracks before your departure.
