To ensure every patient receives safe and appropriate care, you must complete our initial assessment questionnaire before an appointment request can be submitted. During booking, you are required to provide your Full Name, Full Address including Postcode, Date of Birth, and NHS Number.
Once our pharmacy team reviews your submitted questionnaire and details, we will notify you whether your appointment has been approved, if additional information is required, or if alternative care is recommended.
